I've been trying for 5 years now to turn my Moodtracker I built in Excel to help manage my Bipolar into an App but no dice. I have a feeling I'll be able to do it with Power Apps but I need help.
Essentially it works by me selecting symptoms in a form, I have about 100 different symptoms and each symptom can correspond to more than one type of mood. For example "Crying" as a Symptom could correspond to the mood of depression, anxiety or suicide risk.
I have about 10 different moods I track these include Depression, Mania, Psychosis, Mixed Episode, Anxiety, Normal, Suicide risk,
When I fill in my mood tracker it adds up all the symptoms I've selected and gives me a percentage for how many symptoms i've picked for each mood.
For example I may have picked 7 out of ten symptoms for Anxiety which would coincide for 70% level for Anxiety.
I then rank my moods from highest to lowest. So I may have 70% for Anxiety then 65% for Mania, 30% for Normal etc.
I then have a chart which shows me which moods I need to focus on at the present moment, i.e. the ones that have the highest score.
I also then have a list of things I can do to help me with each mood as well as a list of other things people can do to help me for each mood as well.
It works really well in my excel but I just need help geting the initial symptoms to be available to be selected in App form, as I've had trouble using my computer when I get in a really bad state, having a simple checklist in App form that then feeds into my excel would be better.
Hope you can help, not sure how I give access to people to see my moodtracker in excel but if someone can let me know that would be great. Thanks in advance for any assistance anyone can provide.
How do I send data of my selected symptoms back into my excel sheet?
Essentially I want to pick multiple symptoms from my list of symptoms and then send them back into my excel spreadsheet?
Ideally I'd also like to create a recording of the symptoms I've picked with a date and time stamp?

Here's a link to my Moodtracker in Excel.
Its the table Tracker that is where most of the brains of it is.
Essentially all I'm trying to do is create a form that would feed into the Tracker spreadsheet of my Moodtracker of my selected symptoms. The table then has a matrix I've created which crunches the numbers of what symptoms feeds into what mood.
I then have a Final Results Table which shows me where I'm at.
I tried making a database but kind stuffed it up as it only records the totals of the moods when really I want to keep a database of the symptoms I've selected each day. Ideally then I can over time run some regression analysis over it when I have enough data to look for triggers.
Feel free to make whatever changes you like, I have a copy elsewhere.

If it's possible to drop in the location of where I filled in the moodtracker that would be handy also. I could hopefully setup an email alert then for friends to know where I am. If you could simply drop in the lat long into the final database with a date and time stamp that would be brill.
I just opened up the link I loaded up before, in the Tracker table I had setup a tickbox radio button for each symptom in Column 1 of the table. This triggered a true false response which essentially turned column "C" in the table to either a 1 or 0. Its Column C that essentially drives the matrix next to it. There's also the results of the matrix both below the symptons in Column B there is also some more results starting on the same worksheet at column EA:149.
It was trying to get the radio button to work in excel which made it hard for me to make it into an app all these years.
The symptoms themselves are quite long so in Table "Moodtracker Database" I've quickly gone through all the symptoms and shortened them so that can fit hopefully in the app. I've done this in column and titled it Heading. Be nice if maybe that you could have another screen that expands the heading to display the full writing of the symptom.

Looks like it is a bunch of data and some tables are just at the right to the left side table (Not advisable as the Left side table will auto geenrated PowerApps code and will push the right side table and mess-up.)
1. Please have all TABLES in different Sheets (to prevent distortion to table)
2. Please have TITLES for each field (I can see mostly are Column1, Column2, ...). Can really Connect to each other without proper field Titles.
3. There are so many Tables Inside as well as data without table. Which one of them are to be used?
4. No formula or Pivot Table, It must be a pure Raw Data
5. May you create a flow on how do you want to accomplish and make a link (forum here is more on solving problems on formula. For the whole project, it is better you make a Flow how you want the Apps to work).

Ok I'm trying to clean up the tables to be pure raw data at this stage.
Essentially the flow is select the symptoms
Symptoms selected then total in a number of moods.
Moods are then ranked highest percentage to lowest
Advice is then given for top 3 moods of what I can do and what others can do.
The symptons selected are then also saved in a database with a date and time stamp.
