Create new item from gallery, in a different list

I have a gallery that displays a list of services contained in a SP list (6 services in the list at the moment).  There's a toggle available to turn each on or off.  There are also other screens that allow you to turn off individual services per customer (different list), as well as disabling customers per service.  

 

This is how I patch the list on the first screen in question:

Patch('Alert Control', LookUp('Alert Control',ID=ThisItem.ID), {State: Toggle1.Value});

What I want to do is add another sharepoint list that logs these actions, as multiple users can turn things on/off and we'd like to be able to audit that.

My issue is that when I add the patch function to the toggle, it's creating multiple records.  I've tried it a few different ways below

Patch('Alert Control Log', Defaults('Alert Control Log'), {Title:ServiceList.Selected.Service,'Changed by': User().FullName, 'Changed at': Now()})

That results in 8 list entries, all with the same service  (recall there's only 6 in the list)

Patch('Alert Control Log', Defaults('Alert Control Log'), {Title:ThisItem.Service,'Changed by': User().FullName, 'Changed at': Now()})

  Results in 7 entries, with the selected service added twice

If(ThisItem.IsSelected,Patch('Alert Control Log', Defaults('Alert Control Log'), {Title:ThisItem.Service,'Changed by': User().FullName, 'Changed at': Now()}))

This is getting me the closest.  Results in 3 entries for the same service when disabling a service, and 2 entries when enabling.

And re-enabling...

What am I doing wrong?  I'm sure I could do it via a flow, but I'd like to figure this out.

I do think the easiest way would be flow, using the "when an item is created" trigger. Nevertheless, you can do it this way and you actually wouldn't need the 'Changed by' and 'changed at' columns doing it this way, because sharepoint lists automatically capture those values in the 'created' and 'created_by' fields.

I think this might be an issue of the control changing a few times, maybe because of a refresh of the data or a reset on the gallery.

Instead of putting the action on the toggles could you have a "Submit" button which does these actions?
