How to store images in a SP list for a long questionnaire with multiple images per question

Dear all, 

I have to make an audit app, but I'm getting stuck in how to save pictures.

There are 50 questions (e.g., "Is it safe?", "Is it clean?")

- For each question, the auditor has to do three things

1. Choose "Yes", "No", or "Not applicable" (mandatory)
2. Add images as proof (not mandatory) 
   • Sometimes multiple images per question 
   • Preferably both via photo and upload, otherwise only photo
3. Add any additional remarks (not mandatory)

Later, I have to make a report from the inspection, showing the questions and their answers (including the photo's to each corresponding question), calculating a final grade (based on the choice questions) and recommendations (all the comments combined). 

UX could be something like this picture. I see some things on libraries forum posts. Especially for this part of knowing which photo belongs to which question, I think that I need to somehow store the metadata and I just don't know how to make it in Power Apps. I don't have experience with this part of the Microsoft suite (yet), so answers with nocode/low code are highly preferred.

Currently, my best idea is to make a SharePoint list with 50 question columns, 50 comments columns and 150 image columns. Apart from the tedious work, I'm however afraid that this will make the app extremely slow. The app itself will be used <1000 times a year, so the number of rows won't be a problem.

I have a blog that may be useful, however in your case, you really only have two choices - store as attachments or (my recommendation) a SharePoint Library with a linking field to the Parent ID. This process for this in in the blog.

I did already stumble upon your blog, although it was hard to fully understand it for me. For now, I will go for one image/column per question to have the most important image as proof, and for the attachments (as per your recommendation). This seems to be the easiest to implement for now and satisfy enough of the needs.
