Wanted to report that I didn't rebuild my app (though I was preparing).  I did reach out to Microsoft support.  It turns out that SharePoint rolled out some changes over the weekend and rolled them back later on Tuesday afternoon.  My issues are now resolved.

In the PowerApps design studio go to View > Data Sources and make sure the "Request" data source is still a data source.

Yes. It is still there.

If you hover over the yellow triangle in your screenshot what is the message that gets displayed?

An error occured on the server
I get this error in a triangle on my form where in Item property there is a syntax like this :

If(IsBlank(SharePointIntegration.Selected) || IsEmpty(SharePointIntegration.Selected),First(Requests),SharePointIntegration.Selected)
And also in SharepointIntegration in Data Source I get the same error.

If you can fix the issue with data source property on the SharePointIntergration control then that should help fix the error on the form. Couple of things to try.

Go to the data source property on the SharePointIntergration and remove the value and then start to type it back in. You should get a list of valid values to choose from after typing a few letters and then choose Requests. If Requests doesn't show up then there is an issue with the data source name.

Go to View > Data Source and find the data source called "Requests" and click on refresh. If the data source isn't called "Requests" then that is probably the problem. If you get an error refreshing the data source then hopefully you get a descriptive error message that will help track down the root cause.

I tried both which you wrote and none of them worked. I can find the the value of data source which is here "Requests" on property of SharePointIntegration. And I also tried to refresh the DataSource by clicking on refresh button. But non of them helped and I don't get any new messages to know where the issue is. Actually the list is available in Sharepoint list and when I make a new connection it fetch all fields from the list.
What I wanna know is what can happen to the list or to the PowerApps that makes a disconnection between list and PowerApps?

I have never seen the SharePointIntegration get disconnected from the list.  I assume you are using O365 SharePoint and that you have access to the list (since you can customize the PowerApps custom form you have the correct access). If you haven't done too much work on the custom form you try deleting the custom form from the list setting page and then re-create it to see if you still get the error.

Hi @Sara_Shah,

I have made a test on my side and don't have the issue that you mentioned.

Please take a try to remove value from the DataSource property of the SharePointIntegration control, then re-type your SP list name within it, check if the issue is solved.

Please also take a try to click "Back to SharePoint" button within the left top corner of your PowerApps Studio, then re-click PowerApps -> Customize forms button to re-create a custom form for your SP list, check if the issue is solved.

In addition, you could also take a try to re-create your SP list, then re-customzie a form for your SP list, check if the issue is solved.

More details about customizing a SharePoint list form using PowerApps, please check the following article:

Customize a SharePoint list form using PowerApps
