I am new to Power Apps, and would like to look at building a Barcode scanner app, that also has the ability to upload documents
I know that I can configure the Scan & Log button to capture data in the Fields on my canvas and don't need to display the columns such as Title, Time & Who Scanned within SharePoint using Formulas, but I want to be able to upload documents as well as Scan.
Is this possible?
Can anyone send me screen shots, or help me achieve the ability to be able to upload documents and scan a barcode?

Could you please share more details about your scenario? What documents would you like to upload? What information would those barcode hold?
As per your screenshot I assume that you are using customize form of SharePoint list. If you want to upload documents you could use the Attachments control to add documents you like. If you would like to fill the Item field with that scanned barcode, you could set the OnScan of the "Scan & Log" button to
Set(varScan,Self.Value)
and set Text of the Item box to varScan
Please provide more details about your scenario if I did not provide the proper solution.

Thank you for coming back to me.
I'm looking at the options at the moment, at this is the first time I have used Power Apps, and certainly with a Barcode feature.
The aim is to be able to do the following:
An idea that I have been given is to have a decision box pop up to confirm on of the stages mentioned above, attach documents, word, excel, pdf, drawings, images. all of this logging the information back to SharePoint list(s).
I thought that I could have fields on my form that when the barcode scanned, the users could either upload a document, enter some text/information.
Any suggestions on how, would be appreciated on how to connect my SharePoint Columns with data, and scan the barcode.

You mentioned in your post to apply as formula in the OnScan.
I already have a Patch formula set, is there a way to add in the Title and Attachments to also fill in using the patch formula, I have tried but keep getting an error.

What is your data source? As per your screenshot in original post I was thinking you are working with SharePoint list customize form, but in the last post the screenshot made me suspicious.
If you are working with SharePoint list customize form, it would be better for you to submit the item with the Form. In your SP list add columns "Item" and "WhoScanned", then in the customize form add all those fields you would like user to fill in, and the Item DataCardValue Default property set to the variable "varScan" which I mentioned in my last post.

Thanks for the reply, I've managed to do what you suggested, but I now have another issues which I'm not sure if you can help resolve,
I tried to use the App on my phone, and I get this message
I would like the user to be able to use this App on a mobile devise, is that possible with a Customised Form via SharePoint,
Any help is appreciated.

Like the prompt says, Barcode Scanner only supported in a STAND ALONE PowerApps App when working on mobile phones. So, even though on mobile phones, Customize Form in SharePoint working on Web Browser is not supported.

I've not used Power Apps in this way before but have been asked if its possible to scan a Barcode and update information into a SharePoint list, with information and documentation

If you would really like to scan barcode then upload information along with attached files, I think a stand alone Power Apps app could achieve that. Given that you are working with SharePoint, does anyone that would use the app you would create to scan and upload have at least an O365 license? If so, with 0365 license, you could create a standalone PowerApps app and others could run the app to achieve that goal, otherwise they will need a standalone PowerApps license to run the app.

When creating the stand alone app, you could generate from SharePoint list and do some customization for instance add the barcode scanner button and like I said in above posts, get information from it:
