I created a form in PowerApps connected to a dataverse table. When users use the attachment feature and attach multiple files, my workflow will only add the first file from the dataverse into my SharePoint document library.
If you look under the Apply to Each, you will see Show 1 of 1 when it should be 1 of 4 or 1 of 5.

Looking at it, it only shows it has 1 attachment. I cannot say why it says that, not exactly but the way it should work is each update would trigger and technically I would suspect 4 triggers.
That or either
a) they all fire in a way so it looks like its only 1 attachment each time since they are simultaneously fired.
or
b) you have gremlins.
or
c) I would try setting your trigger, under Settings to have Concurrency On and = 1. Now I dont want you to have this forever, I just want to see if it ever triggers multiple times (in a row serially or not)

Its 2am for me so I am out, but I will try to repro this tomorrow for you. Is there anything special you can think of about your tables / columns I just want to make sure I repro it as accurately as possible.
Also, and truly im tired so I could be remembering wrong right this second, but we only allowed 1 attachment per row, unless you added special activities so you could add more.
Can you do me a favor and paste the Dataverse side, where you can see the 4 documents? Its possible that you already are using SharePoint integration or several other ways to get around this. I just want to make sure.
And last question. When it triggered. Did it provide you the name of the document it just attached (in the single attachment) or an already existing attachment?

Lets sort of reset then because you didn't mention that you already had SharePoint integration on so.
My question was related too, if you attach a file, it should already be going to SharePoint if you are using SharePoint integration. There should be no reason to create a flow to put it in SharePoint.
So then let's not worry about DT and it triggering and you only seeing 1 file in the Attachments. The question is, when you attach, do you see the files end up in the SharePoint document library, that you are using for the SharePoint integration, with Dataverse?

My apologies @FLMike ! I just added the SP Integration a few hours ago.
Here is my Form in PowerApps with 3 attachments:
When I view the record in Dataverse, I can only see one attachment.
And the SharePoint Integration Library I built doesn't have any of the attachments.
But, if I add attachments here, they show up in my SPI Library.
