I had that same issue. I just waited a few hours and rewatched the video and followed along again and the second time my data loaded.

I had the same problem the first time. Next, I deleted the form, excluded and included the fields as the exercise said, and also moved them up and down the field.
Apparently, when the order is changed it generates a dependency field requirement.
You can see the dependencies related in the field
Delete the fields and add them again in the order required.
If you already deleted, the fields and form and it still doesn't work. Delete the checkout screen and start again from there. Ensure to follow the video in the same order. This was what I did and it worked for me. Better than being stuck and unable to proceed. Good luck!
Speaking of dependencies, after deleting my checkout screen I also removed the formula below from the Checkout Book button (On Select) and clicked save, before creating a new checkout screen.

Then after creating the new checkout screen (follow in the same order as the video to add form and fields), go back to the home screen and input the formula in Checkout Book button. Ensure that you choose 'On Select' from the drop down list on the top left as shown in image below, and that the formula is written correctly. I hope it works this time.
