Try this:
if(empty(items('Apply_to_each')?['Noofassignments']),1,add(items('Apply_to_each')?['Noofassignments'],1))

If Noofreplacements field has the same type as Noofassignments, use the same formula.
Also if you wish the fist time status should be 0 use
if(empty(string(items('Apply_to_each')?['Noofassignments'])),0,add(items('Apply_to_each')?['Noofassignments'],1))

Are you able to post a larger image of your flow - the one you have is very small and difficult to read?
Also, can you run your flow and take a screenshot including the action that fails?

Much appreciated. Fantastic
I got another counter which is Noofreplacements that took place for staffs in the last 12 months, so whenever there is a increment in the Noofassignments counter this should get increment
How to reuse the below in another update action the same flow, do I need to copy the same formula in same flow but different update action and in the Noofreplacements should I add the below expression
if(empty(items('Apply_to_each')?['Noofassignments']),1,add(items('Apply_to_each')?['Noofassignments'],1))
Sorry for inconvenience, Issue is not sorted and i am facing the error again when 2nd time status changed as Active
When status changes 2nd time as Active I'm facing below error
if(empty(string(items('Apply_to_each')?['Noofassignments'])),1,add(items('Apply_to_each')?['Noofassignments'],1))

Fantastic.
Much appreciated. My error got sorted and it got fixed finally.
So, I am getting the expected value whenever status value is modified as active .
Last but not least but there is one small dependency associated in this context of achieving this objective is that when first time status is active then Noofassignments is 1 then Noofreplacements should be 0 etc. or so on but I'm not sure if this below formula works if in it got to updateitem for Noofreplacements field where I need to increment Noofreplacements field and appreciate if you could please consider to shed some light for Noofreplacements
Please correct if there is any issue with the below formula if I put in Noofreplacements fields
Noofreplacements field expression in flow as below
If Noofreplacements field has the same type as Noofassignments, use the same formula.
Also if you wish the fist time status should be 0 use
if(empty(string(items('Apply_to_each')?['Noofassignments'])),0,add(items('Apply_to_each')?['Noofassignments'],1))

Right,i got the same Number data type for Noofreplacements too and I used the same below one but its value is getting incremented twice for second time for Noofreplacements fields when Noofassignments is 2 but in this occurrence Noofreplacements should be 1,please refer below one.
if(empty(string(items('Apply_to_each')?['Noofassignments'])),0,add(items('Apply_to_each')?['Noofassignments'],1))
Post running the flow i am getting below values for 3 times active status got modified
Thanks a lot for your inputs and it worked as expected. Fantastic solution for two requirements.
