Get page address from page with embedded form

Hello.  I am new new Flow, and hope that that someone may be able to tell me if/how we can get the url of a page that has an embedded form.   

The scenario is that we need employees to acknowledge that they have read content (changes to work procedures, one point lessons, etc.).

Tools used:

• Microsoft Form - to gather response from employee
• SPO Page - this is created from a page template that has an text web part as well as the form embedded 
• Outlook - launched when the page is published, and could be sent to random employees
• Flow - copy form response to SP list

We'd like to use the same form for each page, and gather all of the employee responses in the one SharePoint list, but need to distinguish between the content that has been acknowledged for auditing.   

Is it possible to get the page url with the form response to the SharePoint list?

If possible, if you could guide me through the flow with images, that would be super helpful.

From your scenario, the trigger is when someone submits a response to MS Forms, right?

If so, you don't need get the sharepoint page url, because each flow can only be bound with one MS Forms Survey.

You will know where the response comes from.

Thank you Michael for your response.    The flow is bound to the one survey, and it is that same survey(form) that we have added as a web part to a page template.   So when the Author creates a new page, they choose the template with the form already embedded.    They add their content to the page, and publish it.   The select "Email" when prompted, and add the appropriate recipients, along with any message they want to provide.

Employee gets the email, clicks on the link reads the content, chooses whether they agree/disagree, and then clicks the Submit button on the embedded form on that page.    Employee gets message that their response was submitted and then they can close the page.

The flow then takes that response and sends it to the Communications Log SharePoint list where all responses from each employee and each page are stored.     But, what I don't get is the actual name of the page that the employee was on when they submitted the form.   I need this to keep track for auditing purposes.

Basically we are looking for a solution to document employee's acknowledgement of their awareness of content.    Using modern pages, is great in that it is easy to use, and allows you to upload a document or add straight text / images.

I hope that explains our requirement more clearly.

@khubbard looks like my last reply wasn't updated. I understand that all the sharepoint pages are refering to the same MS form, correct?

If so, we can't distingush them as Flow can only get the data from MS form property or response.
