Get the name/title of a Form for use in a SharePoint list

Hello Community.    I have a number of MS Forms, and I am using PowerAutomate to send the results to one SharePoint List.   I need to capture the name of the Form to sort responses in SharePoint.    How can I get the name of the form to populate in SharePoint?   Thank you for any and all help/guidance.

Hi there.  Are you using regular Forms? Or Forms Pro?  We might have a chance w/Pro.

As far as I know, only one Form submission can be monitored in each Flow, so the Form set in each Flow is determined. You only need to configure the Form name in the specified field. I hope that makes sense.

Hello @edgonzales .  We are using standard Flow.    What I'm actually trying to accomplish is to build an employee acknowledgement tool  using SPO pages that have a form embedded in a web part.    Initially, I was looking to see if I could embed one form in a page template, and get the page address from the page, and using Flow, send the form response information, including the page url to a SharePoint list.    I don't think that is possible:

So now, I'm am looking at creating separate forms for each unique page (could be a safety meeting, process change, etc.), and using Flow, send the form details to one SharePoint list.  It's important that we gather the responses all in one place for ease of auditing.   I would copy the flow for each form, but I need to grab the form name.    I was hoping for as little manual intervention as possible.     

 

Any ideas or have you seen any other solutions where you gather an employees "sign-off" in M365?  Beauty of SharePoint pages, is they are so flexible wrt content, they roll up nicely in a News web part, and can be emailed.

Hello @v-bacao-msft .  Thank you for your time in responding.    I can create a flow, and send it to a SP list, but I can't seem to get the name - only the form GUID.    I need the name for this solution to work properly and with as little manual intervention as I can get.

I am afraid that the output of the trigger body and the Get response details action does not contain the Form name.

 

But as I mentioned before, since one Flow only monitors one Form, then the Form involved in this Flow is determined, so the Form name is also determined. I don't quite understand why you are entangled in this problem. Even if you enter the Form id, this id is the id copied from the Form portal. There is also the Form name you need.

He wants to create ONE list and write the values to it from several different flows... same as everyone else has wanted to do for years! In this list, he wants to populate a column that says "form name" so submissions to lots of forms can easily be tracked in one SharePoint list. Telling an employee's manager that his employee completed "BpmoVRDHa0O8RExZDLZ8SoeI-CobBdpLhZpElgiWItBUMkkwSVJXSlpRSUNJQVJHR1RQQllaM0tTMCQlQCN0PWcu" is not going to be particular helpful 😞

 

Yes, we could maintain a lookup table or an array variable to store this but life is too short to fiddle with maintaining this kind of crap in my opinion...

We are excited to announce the Summer of Solutions Challenge!    This challenge is kicking off on Monday, June 17th and will run for (4) weeks.  The challenge is open to all Power Platform (Power Apps, Power Automate, Copilot Studio & Power Pages) community members. We invite you to participate in a quest to provide solutions to as many questions as you can. Answers can be provided in all the communities.    Entry Period: This Challenge will consist of four weekly Entry Periods as follows (each an “Entry Period”)   - 12:00 a.m. PT on June 17, 2024 – 11:59 p.m. PT on June 23, 2024 - 12:00 a.m. PT on June 24, 2024 – 11:59 p.m. PT on June 30, 2024 - 12:00 a.m. PT on July 1, 2024 – 11:59 p.m. PT on July 7, 2024 - 12:00 a.m. PT on July 8, 2024 – 11:59 p.m. PT on July 14, 2024   Entries will be eligible for the Entry Period in which they are received and will not carryover to subsequent weekly entry periods.  You must enter into each weekly Entry Period separately.   How to Enter: We invite you to participate in a quest to provide "Accepted Solutions" to as many questions as you can.
