Power Automate Flow error "An action failed. No dependent actions succeeded."

Hello Community, 

I am a new Power Automate user and would greatly appreciate some guidance. I have looked everywhere for a solution and am stuck.

I created the flow below to take Forms responses and add to a SP list. The flow will add up to five separate rows in my SP list based on Form responses. My flow correctly adds new rows/ list items in SP regardless of how many responses I have; however, I receive this error code any time the answer is "No" on one of the form questions. Does this error even matter if my SP list is being updated correctly? How can I correct the error? 

For example, if someone answers "No" to my form question below, the flow will still create the new list item but Power Automate will say the flow failed. The current condition in flow is if yes, create item. I have not added anything for "if no" condition.

@bstapp you shouldn't have the get response details inside an apply to each. I suspect you created the flow from the Forms template and it puts in that illogical and erroneous apply to each which can cause problems (it doesn't always). It looks like you might have multiple get response details which is not a good idea. Best practice is to create the flow from blank and immediately after the trigger add the get response details action. You can then add in parallel branches and add the condition and create item actions in each branch.

Thank you very much for taking the time to respond. I tried creating a flow from blank and adding a parallel branch as you suggested but when I do dynamic content is not appearing from my form. What am I missing?

What @RobElliott was saying is more like this:

 Then you can add parallel branches to do the calculations at the same time. But with forms the first two steps should always be as in the image attached.

Thank you both. I've set it up as suggested but I'm not sure what to put in the Response ID. When I select "List of response notifications Response ID," Get response details changes to Apply to Each

I wpuld re-create it from blank. Does the form have a multi-select question by any chance as that could be the cause of the apply to each?

Yes, The form has several multi-select questions (2,9,13,17,21) When I was putting the form together I wanted the user to have up to 5 types of material that could be captured per form. 

Questions 9,13,17,21 are identical, but I have added a "*" at the end of each so when I mapped fields in Power Automate I could easily identify them with Dynamic Content. If any of the responses in questions 9,13,17 or 21 are "No" the form finalizes .  This was my first ever form so I wasn't sure if there was a better way to organize it.

Hi All, Still having difficulty with this flow. I have tried building from scratch but when I try to add the "get response details" step the the step changes to an "Apply to Each" regardless of what parameters I select for the Response ID. I am also getting an error message. I do have several multi-select questions in the form previously described in the feed but can't figure out a solution.
