It successfully displays 'Created by' but yet not showing who created the leave request for some reason. How do i need to connect 'Created by' field from my Form with Sharepoint list?

Judging by your screenshots, this may not be the reason.
Did you create the "Created By" manually? Could you find another column like "Created By" column in hide columns?
Please check that you are using the manually added column instead of the SP's own 'Created By' column. Usually, this column is automatically populated when the item is created and is not editable, you can see that our edit column interface is different, your interface is similar to the manually created column. Your Form may display the 'Created By' column, but your list displays a manually created column.
I am not familiar with Share Point and I'd love to hear from others here.

I have form with fields identical with fields from my sharepoint list but when i create leave request here
I dont see any requests then because it filling all fields in sharepoint list except 'created by' -
How am i supposed to fix my application to force it to filling field 'Created by' as well?

If you are using original 'Created By' column of SP list, I recommend you go to list setting and check the "Show field" of 'Created By' column because some field will show blank in SharePoint.
I recommend you select Name.

The challenge is open to all Power Platform (Power Apps, Power Automate, Copilot Studio & Power Pages) community members. We invite you to participate in a quest to provide solutions in the Forums to as many questions as you can. Answers can be provided in all the communities. Entry Period: This Challenge will consist of four weekly Entry Periods as follows (each an “Entry Period”) - 12:00 a.m. PT on June 17, 2024 – 11:59 p.m. PT on June 23, 2024 - 12:00 a.m. PT on June 24, 2024 – 11:59 p.m. PT on June 30, 2024 - 12:00 a.m. PT on July 1, 2024 – 11:59 p.m. PT on July 7, 2024 - 12:00 a.m. PT on July 8, 2024 – 11:59 p.m. PT on July 14, 2024 Entries will be eligible for the Entry Period in which they are received and will not carryover to subsequent weekly entry periods.
