I have taken the integrated SharePoint List and Customized it using Power Apps.
I can save and publish the App without issue.
When I navigate back to SharePoint and perform a New option, I can submit successfully. However, if I perform another New option, the form is stuck at loading, with the moving dots.

Thanks for getting back to me. I reached out to Microsoft Support and the issue ended up being an expired token for that session that would not allow the canvas/app to load. It was corrected on the back end.
I will include more detail as to what the original issue was in case anyone else is having that issue:
-Our environment is SharePoint Online
-I have a SharePoint list that I created
-I selected the ‘+ New’ option, from within Google Chrome, to enter an Item
-While the ‘+ New’ Item is opened, I chose the ‘<><> Customize’ option
-This opens up in PowerApps where I customized the Pre-Designed Integrated form so that it now becomes customized.
-I then saved and published any changes and went back to the SharePoint list
-I then refresh the SharePoint list page
-I then chose the ‘+ New’ option and this opens up the New Item form I customized
-I am able to enter all the fields in the different navigational screens and then save and submit the item successfully.
-The submission page doesn’t go back to the first screen, so I refresh the SharePoint list so that when I select ‘+ New’ Item, it will open on screen1
-However, at this point instead of opening on Screen1, the form opens but never loads the canvas
-The form opens with the spinner that is constantly spinning, with the 3 dots darting across the screen.
-So to me, this was either a case of trying to connect to the data source or building the canvas via .net
-However, to rule out connecting with a data source, I navigate to another SharePoint list that wasn’t altered with PowerApps (The Built-In Integrated form) and it was opening without issue
-I also tried logging out and logging in and still the same behavior
-I also tried different browsers (Edge, IE 11, Firefox) and the same behavior was present
-The only way I could get it to load is if I made a small change and republished my app within PowerApps
-Only then would it load the screen upon selecting the '+ New' option
-Any subsequent browser refreshes would not allow the app to load until I republished
-So in order to do any entries, I had to keep going back to republish before selecting the '+ New' option

Which browser do you used to custom form for your SP list?
I have made a test on my side (within my Chrome), and don't have the issue that you mentioned.
Please consider take a try to clear cache of your browser, then sign in your SP list, try your custom form again, check if the issue is solved. Also please consider try your custom form using different browsers, then check if same issue occurs.
In addition, please consider take a try to remove the custom form from your SP list, then re-create a custom form for it, then check if the issue still exists.
Note: Please consider take a try to close other threads within your browser, to make your browser in a good state rather than a burden state.

hi can you share how to reeachout to a Microsoft Support regarding this matter? Currently I have the same issue but mine is inside the canvas in Preview Mode.
Hi, I'm experiencing the same issue and I'm all the way in South Africa, my business relies highly on these Apps that I have created and it's 2022 but we are still experiencing the same issue that most users(on these replies) have experienced about 2 years ago.
