Setting button missing in MS teams, Inspection app

I'm new to the app of Inspection in MS teams. I've follow the steps on YouTube, everything was fine when first setting up, I created few temp inspections to give it a run, but after I went through and want to edit my inspection forms on "manage inspections" page, my edit button has disappeared along with the option of "inspection forms". My account is the "owner" for my team and channel. I upload screenshot from showing video and my screen as comparison

Could anyone help me with it !

Will the Edit button show up after clicking the Cancel button? I can see when clicking the Edit button on my end, the screen that same as yours then shows up:

The Visible properties of the Edit, Save and Cancel buttons control the visibility of them as well as the Display Mode of the controls within the Screen.

There could be a misleading from my description, sorry about it. 
once we install the INSPECTION app there will be three task bar shows add in channel, which are "Manage Inspection", "Review Inspections" and "Inspections". However, after first set up and run a inspection, I'm not able back to "Manage Inspection" by click task bar, it auto switch to "Review Inspections". 

After keeping delete and install the app, I'm able to back to "Manage Inspection" page but not able to back to "Inspections".  Seems like a bug occurs after fist inspection run, it will randomly change one of the access of these three task bar.

I have the exact same issue, but my settings icon is visible. However, I am missing Locations, Inspections insights and Inspection forms.

I am also owner of the Team and the Channel.

I have tried the following:

• Uninstall and Re-install the app on the channel.
• Uninstalled and Re-installed teams.
• Cleared following folders before re-installing Teams:
  
  • AppData\Roaming\Microsoft Teams
  • AppData\Roaming\Microsoft\Teams\blob_storage
  • AppData\Roaming\Microsoft\Teams\Cache
  • AppData\Roaming\Microsoft\Teams\Code cache
  • AppData\Roaming\Microsoft\Teams\databases
  • AppData\Roaming\Microsoft\Teams\GPUCache
  • AppData\Roaming\Microsoft\Teams\Local Storage
  • AppData\Roaming\Microsoft\Teams\tmp

Stil looks like this after fresh Teams install and new install of the app inspections:

However, in the preview mode in Power Apps, it shows as normal and works fine..

Initially I thought the Inspection have disappeared as it was only loading a blank page. It happened to me twice.

I set up previously and tested it was working fine, but after a couple of weeks of inactivity, the Inspection page wasn't showing the Perform an Inspection action and the stats of overdue, assigned to me, or active items. I was sure I didn't do any changes, and the Review and Manage Inspection sections were still there. So I had to uninstall everything, reinstall and reconfigure. Tested again and it worked. However, after a couple of weeks (maybe another 3-4 weeks), I noticed the Inspection loaded another page, either Review or Manage Inspection. 

Since the old configuration was quite a lot, I reinstalled for the 3rd time but this time retained the existing configuration. While the previous config in the Manage Inspection and Review Inspections were still there, the sample ones (which we get after installing) were added. Now the new Inspection link points me to the correct page (with Perform Inspection button). But then the old Inspection link points to Manage Inspection or Review Inspection, or vice versa. Worse is after several toggling, my Manage Inspection is now not loading; it points to Review Inspection or Inspection instead 😞

Terrible bug.
