when in PREVIEW screen says "No item to display"

I'm creating an App in Teams. When selecting the Preview option, I get "No Item to Display". The default for the form is set to "New" (see below).  I have the form within a Container. I'm just beginning to learn this so I'm new!

A new form will not display data (it is waiting for input so a new item can be created). It needs to be Edit or View to display a record.

Thank you for your response - Let me clarify - in the bottom screenshot you will see the where it says no item to display (in PREVIEW mode) The top screen shot is to show the settings (this is NOT in Preview mode).

Actually, you do have an issue there as preview should show empty controls.

What is the Item and DataSource of the form?

It should display a blank form (empty controls) - that is my question. I believe I fixed it as its working correctly. I'm not completely clear on what I did, but its working now. However there are some things I'm not sure on. It loads very slowly. 

To answer your question :

For the form - Items - has no value. DataSource has the name of the sharepoint list - "Status List"

The form generally needs an Item or it does not know what record to display, but if you are only using it for new records, this should not matter.

When you set the mode to New, it will start with a blank form, but if you then submit the form, it will go blank.  Make sure you have a resetform on a button or the screen's OnDisplay.

Thank you - I forgot to look out for that and how to do it. When submitting a form, "onSuccess" I have it switch to a "Success Screen". Currently I have a BACK() function on a button to go back to the previous screen. This success screen is used for several forms I have on the app. Is there a way to use the "ResetForm()" function to go back to the last form used? Or do I need to hard code it for each form? I would think I could store the current active form in a variable and use that, but I'm not sure where to put that.

Thank you for your help. I have another question. When using the "RunOnStart" function, I have this : (But when I then click on "Preview" the form " Status Form" (in the Status Screen) is blank and the default is set to NEW.
