PowerBI Pro is only required for the users who will use the Location Readiness Dashboard and Facility Safety Management App.
PowerApps license are required for everybody. We will mention this more explicit in the docs.

Everything seems to work for me in our environment except adding new facilities. I get a script error. below are the details

I also get error loading control under New Facility. Other stuff seems to work. PowerBI, Check in App
Verify if you have legacy form rendering turned on under Settings - Administration - System Settings. Make it false.
What would the monthly cost per user that would use this system to report health status be?
use legacy form rendering is off. I'm just re-reading the deployment guide. Is it it due to the embedded PowerBI report? I enabled that, but didn't actually do all of step 5 Embed the Power BI report in the model-driven app. I think that may be my actual issue so let me work on that.

I was able to embed my published report using XRMToolBox PowerBI Embedder. The form shows the report, but I'm still getting error loading control and the gerProcessManager.
Legacy form rendering is off.
Any other help I'd appreciate it. Thanks!
