Here is a copy of a flow i built. The purpose of the flow is to have the person who enters an item receive a confirmation email when the item is created, and a follow up email when an item is modified.
The flow runs fine when we enter a new item but fails when we modify it.
Not sure why?

The flow up to this point works perfectly. The next segment does not work
• When an items is created or modified

• Condition: Created does not equal Modified
• If yes: Switch
• If no: nothing happens
On PHASE = 9 different cases
Each case is set up with
• Get items for the specific phase selected
• Send an email
The flow shows that it is running when the phase is changed but it never completes. Any ideas?

I did 2 things. I changed PHASE append to PHASE value and i removed the GET ITEMS from each switch case and now i have a perfectly running flow. Thank you so much for hanging with me through all the trouble resolution. This is my first flow and i have learned a great deal from your help.
I believe i might have corrected it from your first recommendation. I went in and re-established the connection and it looks like that fixed it.
A screenshot of the flow would be much more helpful than a list of the steps. Could you provide one?

Thanks for providing the screen shots.
Do you want to do certain actions if the item is newly created, while other actions if it is modified.
I would suggest you remove the first segment and use the trigger When an item is created or modified directly.
You could add a Condition to check if Created date time equals to Modified date time, if yes, it is a new action, do actions you want. If not, it is a modified item, do other actions.

OK i did what you recommended and the second part runs now with success but returns the wrong result. It returns a result of default instead of the appropriate pathway in the switch. I do not have default set because i am using a choice column that forces a choice between 9 items.
i completed the build of the flow as you advised. It runs successfully but delivers a default response which is incorrect. The switch offers 9 choice options and does not offer a default.

Could you share more details on variable “var PHASE”?
The action Append to string variable select output from “PERSON REQUIRED”, While the value is dynamic content PHASE. Could you shed more light on this part?
I have set up a simple flow likes below for your reference.
Further, from the screenshot it seems that Switch 2 is added between the actions. Could you please try to add it at the top within the If no branch?

Per you note above.
The var PHRASE is = to the column in my list titled PHASE. This column is a choice column where the user can select one item in the column choice. There are 9 choices in the column. As the item is processed the user will come to the column and changes the phase. Each time the phase changes we want an email to go to the person who created the item
It is not “PERSON REQUIRED” It is “PERSON REQUESTING”. This is the email for the person creating the item in the list. The "PERSON REQUESTING COLUMN" is a People or Group column that is a selection form our active directory that has the persons name and email address.
the screen shot is a bit off. The "Switch 2" action is directly at the top of the "no" side of the "condition" On the yes side of the condition i have an action of send email. This is to send an email to new items created on the list. On the no side of the condition i have the switch to send an email when an itme is modified based on the phase selection.
The flow is running successfully but will not select the correct switch chosen. It automatically goes to the default switch and does nothing as the default switch is not set to do anything.
i just noticed something else i think is affecting the flow.
when i add the send an email from a shared address under the yes side i get a mailbox address field option. when i send an email v2 i do not get this option. The yes side of my flow is running perfectly yet i am using the var Report Request in the address field as our company has not turned on the option to use our share email boxes yet.
You mentioned that the PHASE is a column of type choice right? In that case, in the variabel the value of the choice should be intialised (in the dynamic selector it will appear as PHASE Value). Can you confirm that this is what you are doing in the flow? Based on your comment, that the default case is being selected, I am guessing the issue could be due to this.

I have already initaialized "PHASE" as a string variable labeled "var PHRASE. However when the dynamic content comes up it does not offer me an output choice of phase or phase value. See below.
The cursor is placed in the Apply to each action. Can you place the cursor in the Initialise variable -> Value ->and post a screenshot of the PHASE option coming up (or whatever it is that is coming in the dynamic selector)

Not this, so you click into the Value option of the Append to String variable action. Now in the dynamic selector, type phase and take a screenshot of that (I want to see the list of options coming in from the get item or any sharepoint action)

My flow was running very nicely. Then without any changes to it i got a notification it had stopped running. When i open the flow run dashboard it says there is a problem with my triggerr.
When i open the flow it says my trigger is "unauthorized"
