Hi All,
I have multiple flows that are working fine. But since yesterday, when i create a new flow (copy an exisitng one), it fails to run when the action happens. This particular one i am trying right now is
- when a new response is submitted on MS form
- run the flow
Since its not running even once i cant manually run the flow either. Can someone plese help how i can debug this issue? Also is htere a known issue going on that is causing this problem?

My previous flow (created in 2019) working with the same architecture as you.
But when I create a new flow based on a form answer, I have no response to my flow even if I'm waiting several days.
I have critical process based on this system for my team.
Someone has a solution please or a Microsoft teams worker can answer to me thanks.

Having the same issue still in 2021 - simplest flow has been working fine for months and now get this error:
CDB TableNotFound. Inner Message: {"error":{"code":"NotFound","message":"Table 'T2KLEY2MK6RDE4XMP7A3XUROES' cannot be found","innerError":{"code":"TableNotFound","params":["T2KLEY2MK6RDE4XMP7A3XUROES"]}}}
I don't have a good answer for you as to the "why". That said, since this appears to be a reasonably simple Flow, I suggest recreating the Flow from scratch and turning off the existing Flow.

This does seem to be a long running issue with Flow - does not inspire much confidence.
I am also now having the same issue. Form to SharePoint list flow was working fine yesterday. Today it will not trigger even after rebuilding it from scratch..
I deleted all the copies of the form, and other forms not in use, and then the form I was trying to use as a trigger became available in the dropdown. But one the flow was created, i could not get it triggered by a submission

Confirming that this is currently a new issue for my flows as well. Two flows triggered by form submissions were working from May 2022 until 13 Feb 2023, but no longer working as at 16 Feb 2023 with no changes and no clear explanation.
I'm having the exact same issue.
Forms submition not being detected by powerautomate.
I've tried to create a new flow, but then I cannot save the flow, it givers me a CDB error.
Same issue, No error, when I run an automatic test in power automate on a previously submitted form it is successful but then when I submit a form the automate isn't triggered to forward the email. I haven't tried to create a new flow or form yet though, I would like the old ones to work!
Forgot to mention my 3 flows all stopped working over the weekend and they have been working for a year.
I JUST TRIED TURNING THE FLOW OFF AND BACK ON AND IT WORKS NOW
Confirming turning the flow off and on worked for us to get it to start processing responses again.
That said, our form was still collecting data and not processing into SharePoint, and we need to re-process those submissions. At a glance, I am not seeing an easy way to do this since the Forms connector doesn't have a list responses feature or anything like that.
Has anyone had any luck with reprocessing missed responses due to this bug?
I quickly tried to find a way to push old responses through, but didn't see an obvious way so I just downloaded all the responses to excel via the form to see which ones were missed and then collected the data I needed that way.
Change the second step (Response ID) using the response ID visible in the forms result page.
Do this for every missed entry, and run a test of the flow with each one

We invite you to participate in a quest to provide solutions to as many questions as you can. Answers can be provided in all the communities. Entry Period: This Challenge will consist of four weekly Entry Periods as follows (each an “Entry Period”) - 12:00 a.m. PT on June 17, 2024 – 11:59 p.m. PT on June 23, 2024 - 12:00 a.m. PT on June 24, 2024 – 11:59 p.m. PT on June 30, 2024 - 12:00 a.m. PT on July 1, 2024 – 11:59 p.m. PT on July 7, 2024 - 12:00 a.m. PT on July 8, 2024 – 11:59 p.m. PT on July 14, 2024 Entries will be eligible for the Entry Period in which they are received and will not carryover to subsequent weekly entry periods.
