Hi,
I have 5 different work flows in place for each of our departments that are each triggered by a Microsoft Form Result when submitted and have been working perfectly fine for the last 6 months. As of today all 5 have stopped working, I've run diagnostic on all of them and there seems to be no issue with the actual setup on the flows it seems that Microsoft Forms and Power Automate are not talking with each other to create the trigger? The forms are definitely being submitted as coming up in MS Forms results but there is nothing shown in the Power Automate history, no failure or anything.
Could anyone provide some assistance or had similar issues?

We are experiencing the exact same issue here in NZ. Forms submitted and sitting in the response section of Microsoft Forms but no Flows which are supposed to trigger by a submitted form are triggering.
Not ideal.
I have the same issue here in Singapore. All existing Flows that were supposed to be triggered by a submission of a Form is not getting triggered. Hope somebody can shed a light on this issue at the earliest. It is affecting a lot of our forms.

Also experiencing the same issue in Japan.
When I check the flow history, it indicates that the data entered on my form has been "skipped" as the status column states.

Same issue in Singapore. I have been testing it where it works until last Friday. All the flows today aren't working.
I've force testing with successful data and it turns out that Get Profile V2 return 404 - resource not found. Hopefully, this issue can be rectified as soon as possible.
Wonder if it is possible to execute the forms submitted prior to the downtime?

After the hotfix, the flow is working with some delay between the form submission time and the trigger. (About an hour)
I'll watch how things go within the next few days to see if the trigger delay gets resolved.
I hope it works fine by now. You can see about this issue at service health of Microsoft 365 admin center. (if you have access as an administrator)

This issue is affecting me now

I am having this issue right now. Two separate flows connected to two different forms are not starting when forms are submitted. There are no errors on either end - the forms are submitted and recorded on Forms but Power Automate does not launch the flows they are supposed to triggers.

Same issue here as reported by GarciaMcgC. All other triggers seem to work, but Forms triggers are only happening sporadically across all our flows. Started this morning between 9:00 and 10:00 CST.

We invite you to participate in a quest to provide solutions to as many questions as you can. Answers can be provided in all the communities. Entry Period: This Challenge will consist of four weekly Entry Periods as follows (each an “Entry Period”) - 12:00 a.m. PT on June 17, 2024 – 11:59 p.m. PT on June 23, 2024 - 12:00 a.m. PT on June 24, 2024 – 11:59 p.m. PT on June 30, 2024 - 12:00 a.m. PT on July 1, 2024 – 11:59 p.m. PT on July 7, 2024 - 12:00 a.m. PT on July 8, 2024 – 11:59 p.m. PT on July 14, 2024 Entries will be eligible for the Entry Period in which they are received and will not carryover to subsequent weekly entry periods.
