Power Automate Approval Issue

Hi, 
I've created a Leave Request app. I'm trying to create a flow on a powerapps botton click. The flow should send an approval Email to the manager where he/she should be able to Accept/Reject it with a comment. Then the response is sent to the Leave requester via email with the manager response and a Sharepoint List 2 columns are updated with the approval response and comment.
Now the issues i'm facing are,
1. I'm not getting the inputbox for managers comment in the email.
2. While the manager Accepts/Rejects in the mail gets an error "The approval has been reassigned".

I've attached few screenshots, the error i get, the flow structure and the mail received.

Some issue may appear when use approve in the outlook so I suggest that you could make the response in the approval center if you want to add the comments.

I noticed that you are using Start an Approval instead of Create and Wait for an Approval.  Start an Approval is only half of an Asynchronous process.  If you use that action then you need a matching Wait for an Approval to retrieve the response.  Create and Wait for a Approval is the Synchronous version and has both halves in one action.  That may not be what is causing your issue, but that's where I would start.

 

Also, your condition should use 'Outcome' rather than 'Response'.  The 'Responses' dynamic content is a collection.  The fact that you are seeing 'Response' at this level also probably means that you are using an older version of the Approval action that is deprecated.  That may also explain why you are using Start and Approval rather than Create and Wait for an Approval.  Please update it to the current action. I'm assuming you started this with a template. That template is old.

Please take a try with @Pstork1 's suggestion so your problem could be solved.

1. Use the "Start and wait for an approval" action for the approval.

2. Choose the "Outcome" dynamic content from the "Start and wait for an approval" action in the Condition.

Could you please share a screenshot of the run history of the flow?

If you want to send the email with the comment, you should create the flow my screenshot below:

And you should choose the Approve or Reject, instead of the Reassign.

The 1st issue is i don't get the comment option in the Mail(not sure if i've to turn on or change anything in the settings). 
The mail has only 2 options (i.e, Approve, Reject), so i clicked on Approve option and it redirects me to the web where i receive "The approval has been reassigned" error message.
I've changed the order of Apply to each 2 where, if approved update the SP list then send the approval success Mail.

I appreciate for the time and concern.

Is the approver an internal user?  Someone in your Office 365 tenant Azure AD?  Or an external user?  Also, what email client are you using?  External users and non-Outlook email clients can have issues with the actionable cards sent by the Approval process.  But there are no extra settings to get the comment block.  I've also seen situations where you have to pick either Approve or Respond before you will see the comments section. Again, that is based on the email client.

Yes, All emails are on Office 365 tenant.

What email client are they using?  And does the comments box show up after you click Accept or Reject?

We're using outlook app from the store. 
The button on clicked redirects to the flow web where the error message is shown. No Comment box is appeared.

Yes, the flow is successful when we tried it through the app both on web and on mobile phone. There we're able to provide the Approver's comment and works as expected. 

It is on the Outlook Mail where we're facing the issues.

Summer of Solutions | Week 4 Results | Winners will be posted on July 24th

We are excited to announce the Summer of Solutions Challenge!    This challenge is kicking off on Monday, June 17th and will run for (4) weeks.  The challenge is open to all Power Platform (Power Apps, Power Automate, Copilot Studio & Power Pages) community members. We invite you to participate in a quest to provide solutions to as many questions as you can. Answers can be provided in all the communities.    Entry Period: This Challenge will consist of four weekly Entry Periods as follows (each an “Entry Period”)   - 12:00 a.m. PT on June 17, 2024 – 11:59 p.m. PT on June 23, 2024 - 12:00 a.m. PT on June 24, 2024 – 11:59 p.m. PT on June 30, 2024 - 12:00 a.m. PT on July 1, 2024 – 11:59 p.m. PT on July 7, 2024 - 12:00 a.m. PT on July 8, 2024 – 11:59 p.m. PT on July 14, 2024   Entries will be eligible for the Entry Period in which they are received and will not carryover to subsequent weekly entry periods.  You must enter into each weekly Entry Period separately.   How to Enter: We invite you to participate in a quest to provide "Accepted Solutions" to as many questions as you can.
