Open form from a Gallery

My PowerApp is built in a 1 to many fashion where the "Intake Request" is the main record with "Assessments", "Tasks", and "Notes" as the many records connected to the main Intake Request. I have created a screen "Intake Assessment Work Q" that pulls All assessments into the gallery that are not complete. The first (next icon) opens the assessment form up correctly so the user can modify the assessment.

The problem I am having is creating another (next icon)  to open up the Intake Request record incase the user needs to see the original request with additional information.

OK - your Gallery Data Source is 'Risk Assessment' which is a child list to 'Intake Requests'  - what is the "linking" field name in Risk Assessment'  that identifies it Parent record in 'Intake Requests'  ? If for instance you had a field called RefID in 'Risk Assessment'  which was the ID of the parent in 'Intake Requests' , then the Item of your Form would be 

LookUp(
   'Intake Requests',
   ID = varAssessmentItem.RefID
)

This is easily solvable - I just need to know how to refer to the record in Intake Requests'

I will try to set out the logic I am getting from your posts - please correct anything wrong here: -

• Parent List 'Intake Request' has a child list 'Request Assessments' with the field 'Ref ID' matching the ID field in the Parent
• You have a gallery where the Items are from the 'Request Assessments' List (no restrictions on fields available).
• You are setting the Variable varAssessmentItem OnSelect of this gallery to the record selected. 
• You have a Form with the DataSource "Intake Request" 
• If all of that is true, then the Item of this Form should be what I posted and will display the Parent record of the Gallery record selected - you do not need to change anything else.

That opened the form and populated it correctly but now the galleries associated with that form do not populate any information.

Nothing done in the last exercise should have affected these as they refer to varRecord rather than varAssessmentItem, however looking at these you simply need to set varRecord when you select an item from your gallery containing the 'Intake Request' list. Where and how do you set it now ?
